PRICING & HOW TO SCHEDULE SERVICES

Care Packages, Levels & Pricing Overview
Care is delivered in phases. Patients begin in an Optimization Phase and transition to Maintenance once
clinical stability is achieved. Pricing ranges are individualized and finalized at your appointment based on clinical
needs.

PHASE 1: OPTIMIZATION
Active treatment focused on correcting imbalances, resolving symptoms, and stabilizing lab values. Typically
3–6 months.
1 Initial comprehensive consultation
2 Baseline and follow-up labs (priced separately)
3 Medication initiation and dose titration
4 Monthly follow-ups
5 Lifestyle, nutrition, and supplement guidance
Optimization Price Range: $300 – $650 per month
Final pricing determined at appointment based on complexity and medications.

PHASE 2: MAINTENANCE
Ongoing care once symptoms and labs are stable. Reduced visit frequency and lower monthly cost. Patients
may enroll in multiple maintenance tracks.
Maintenance Price Range: $125 – $300 per month

INDIVIDUAL & ADD-ON SERVICES
1 Laboratory testing (per draw): $75 – $425
2 Medication prior authorization support (when using insurance): $50
3 Extended or complex medication prior authorization support (when using insurance): $125
4 One-on-one lab or treatment review (dedicated visit): $125

INSURANCE PRIOR AUTHORIZATION DISCLOSURE
North Iowa Wellness Clinic is a cash-based practice and does not bill insurance for medical services. If a patient elects to use their insurance for prescribed medications, prior authorization services are available for an additional fee. These fees apply only when insurance is used and are not required for cash-pay medications.

PATIENT CONSENT – PRIOR AUTHORIZATION SERVICES
By selecting prior authorization support, I understand that North Iowa Wellness Clinic will assist with
insurance-required medication authorization on my behalf. I acknowledge that prior authorization fees are
separate from medical services, are not billable to insurance, and are my financial responsibility regardless of
insurance approval outcome.

HSA/FSA cards may be used for eligible services. Laboratory testing, medications, and supplements are not included in package pricing unless explicitly stated.
